A more tangible increase shines for junior medical personnel, for whose representatives, from October 1, a level of 80% of the same indicator of the average monthly income from labor activity has been set. That is, if now the average salary of a junior medical worker is only slightly higher than 12 thousand rubles, in the fourth quarter of this year it should increase by about 8 thousand rubles.


In total, according to the regional Ministry of Health, compared with the end of 2016 in October of the current year, the growth of salaries for health workers of all categories in the region should be 7.5%.

When revising wages for certain categories of workers, only their first component increases - salary. This is becoming a serious problem, because the indexation of salaries in 2019 will have a very weak effect on their size, and the accrual of additional payments today is not fully entrusted to the state, but to the management of medical institutions.

Analysis of the current situation


They promised to raise the salaries of doctors up to 200% of the regional average!

Recall that five years ago the President of the Russian Federation issued the so-called May Decrees. According to the instructions set out in this document, as early as 2018, the level of remuneration for doctors should increase to 200% compared to the average regional salary payments.

Regional authorities claim that this order is already being implemented - back in 2017, in most territorial entities In the Russian Federation, the remuneration of doctors became equal to 180% of the average regional salary. Many heads of medical institutions complied with the May decree in a very simple way - they raised salaries and reduced additional payments. Now, in most territorial subjects of the Russian Federation, doctors receive about 30 thousand rubles, and junior and middle medical staff - about 20 thousand rubles.


So far, with salary increases, the real incomes of doctors only fall.

Medical workers representing the Jewish Autonomy, the Nenets Okrug, Ingushetia and the Arkhangelsk Region say they are getting less than before the "increase" in salaries.   • if we take individual categories of health workers (for example, analyze payments to general practitioners), then significant regional disproportions are visible to the naked eye. In the capital, such a doctor receives about 60 thousand rubles, and as soon as he starts working in the region, wages immediately drop to 45 thousand.

Alas, these statistical calculations do not really reflect the current financial situation in Russia. Since 2017, employees of this department have been counting together the average salary of doctors who work in clinics of all forms of ownership. At the same time, salaries in the city hospital are significantly lower than in private institutions of the medical industry. Well, if we compare with the payments accrued to foreign doctors, then the situation looks quite deplorable - for example, American therapists on average earn 416 thousand rubles, French - 200 thousand, and German - 225 thousand rubles.

The government comments on the situation with regional disproportions, explaining it by high allowances in the northern regions. Well, the rest of the doctors need to deal with the local authorities, since the federal budget is formed every year taking into account the increase in salaries. The maximum that can be done is to break the increase into two parts: first, salaries will be increased at the federal level, and then at the regional level, taking into account local coefficients.

We also note that the head of the Ministry of Labor Maxim Topilin assured the public: in 2018, the federal budget will allocate 80 billion rubles to finance the remuneration of medical workers. The capital's mayor said that in 2017-2018, the salaries of medical workers increased by 40%. However, the medical workers themselves refute such information - they say that the average wage for their work in the capital is 64.73 thousand rubles.   • 219.5 billion in 2019.

At the same time, it should be noted the rather disturbing news that appeared in the media not so long ago. We are talking about problems with financing the Compulsory Medical Insurance Fund, which is filled with contributions from the wages of employees. According to the plans, the Fund should be replenished by 1.89 trillion rubles in 2018, by 1.93 trillion in 2019 and by 2.1 trillion in 2020.

However, now the government believes that this is not enough. Already at the beginning of 2019, the Fund will come to a deficit state. To cover it, it will be necessary to allocate 95.5 billion rubles from the budget in the national currency, and in a year - to finance 170.4 billion rubles. Auditors of the Accounts Chamber came to the conclusion that already in 2018 the Fund will not have enough money to pay salaries to medical workers, and the amount of the shortfall will be 43.1 billion rubles.

Earlier, doctors were informed that it was the Fund's reserves that would ensure the growth of their salaries. Now, there is not enough money for the May decrees, since this Fund must finance, first of all, the purchase of equipment for the provision of high-tech medical care. As a result, former Finance Minister Alexei Kudrin proposed to suspend the implementation of the May decrees so that the increase in salaries would not lead to a decrease in spending on the purchase of medicines, ordering the latest equipment and repair work.

Some members of the government have proposed raising the insurance premiums levied on the wealthiest Russians. It is possible that this will help resolve the issue of raising salaries for doctors. Salary of state employees in 2020

The indexation of public sector salaries is part of Vladimir Putin's 2012 election program. The main document is Presidential Decree No. 597, sometimes called “Order 597 of the President: Roadmap 01/01/2018”. Then, in May, immediately after taking office, the head of state issued 11 decrees that dealt with raising the salaries of civil servants and other state employees. Then the President promised to raise salaries by 2020, primarily for teachers and doctors.

Salaries are increased primarily for those state employees who are subject to the Decrees of the President:

Salary increases are phased in. Finally:

  • for university teachers and doctors (health workers with higher education), the salary level should be at least 200% of the average salary in the region;
  • middle and junior medical staff and social workers the level of salary should not be lower than the average salary in the region.

Unfortunately, not all categories of state employees fell under the "May decrees". It is obvious that there are quite a lot of those who have not been raised salaries. Therefore, officials promised not to bypass them and also to index their salaries. Thus, in accordance with the law "On the federal budget for 2020 and for the planning period of 2020 and 2021", it is planned to increase the salaries of state employees (see the table below for indexation rates).

Increasing the salaries of state employees from 01/01/2019

It should be noted that some state employees will have to increase salaries regardless of indexation. Indeed, from January 1, it is planned to increase the minimum wage (increasing the minimum wage), which will affect the salaries of junior and middle-level personnel with secondary education. From 01/01/2019, the minimum wage is 11,280 rubles. This increase in salaries will affect approximately 1.6 million people in the public sector.

Regional decisions to increase salaries for state employees

Decree of the Government of the Russian Federation dated December 28, 2012 No. 2599-r obliges the governments of the constituent entities of the Russian Federation to adopt orders on their regional "road maps" in which to prescribe specific figures for each year to increase salaries. Yes, the roadmap. Sverdlovsk region on salary increase 2020 argued that the average salary of social workers should be at least 100 percent of the average monthly salaries in the Sverdlovsk region. And the Government allocated an additional 2.9 billion rubles from the federal treasury to raise the salaries of state employees in 2020 in the Krasnoyarsk Territory, so that regional allowances and the northern coefficient could be taken into account.

At the end of the year, regional authorities traditionally prepare appropriate decisions regarding salary increases for the next year. For example, in the Lipetsk region, deputies decided to raise the salaries of state employees who are not subject to May decrees by 10% in 2020.
